Report a disease of public health significance
Under the Health Protection and Promotion Act, all health care providers, directors of laboratories, and administrators of hospitals and schools are legally obligated to report a suspect or confirmed case of a disease of public health significance (reportable communicable disease) to their local Medical Officer of Health.

Report immediately if indicated or use the disease of public health significance (DOPHS) notification form, unless a disease specific reporting form is provided (i.e., AIDS, chlamydia, gonorrhea, hepatitis B, hepatitis C, Lyme disease, mpox, rabies, syphilis, and tuberculosis) or fax (613-549-0349).
