The contents of KFL&A Public Health website or resources may be used and reproduced provided that a Request to Adapt or Reproduce Materials of KFL&A Public Health form is signed and returned by the requesting agency, and provided that the agency requesting permission acknowledges KFL&A Public Health as the source of the information.
