KFL&A Public Health has several programs, clinics, and services that health care providers can refer patients to promote health.

The Nurse-Family Partnership (NFP) program is a free home visiting program where a public health nurse visits young, first-time parents during their pregnancy and the first two years of their child’s life. The program can help support parents to have a healthy pregnancy, a healthy baby, and continue to meet their education and work goals. If participants are not eligible for the NFP program, they may be eligible for the HBHC program. To access the Healthy Babies Healthy Children program or the Nurse-Family Partnership program, patients can self-refer. Food for You, Food for Two prenatal program |
Food for You, Food for Two is a free prenatal program for pregnant people and their support person(s). Participants will connect with other pregnant people, learn about food, nutrition, pregnancy, and taking care of a baby. If you have a client that would benefit from this program, please complete a referral form. |
