Records and reporting
In Ontario, health care providers do not automatically report immunizations to their local public health agency. Parents and guardians are responsible for reporting their child's immunizations to KFL&A Public Health to keep records up to date as per Immunization of School Pupils Act standards.

Report immunizations to KFL&A Public Health
- Fill out our online form to upload a copy of your immunization record.
- Submit immunization records online with Immunization Connect Ontario (ICON).
- Drop off a copy of the vaccination record at one of KFL&A Public Health’s office during regular business hours.
- Fax a copy to 613-549-0985 (confidential fax).
- Mail a copy to KFL&A Public Health - Attention immunization team, at 221 Portsmouth Ave., Kingston, ON K7M 1V5.
Tips for finding your immunization records:
- Look for your yellow immunization card at home. Check medical files, baby books, and school or camp forms. Ask your parents or guardian if they still have your childhood records.
- Contact the clinic(s) where you were immunized (e.g. primary care providers, travel clinics, walk-in clinics).
- If your health care provider retired or moved, you may be able to get help locating records by contacting the College of Physicians and Surgeons at 1-800-268-7096, ext. 603.
- Contact schools, colleges, or universities that you attended.
- Contact previous employers, or the military if you served.
View your immunization record online
To log in to view your records online at Immunization Connect Ontario (ICON), you will need your health card number. If you need support, or need an Ontario Immunization Identification Number (OIID), please call KFL&A Public Health at 613-549-1232.
If you cannot access your records online you can request to pick up immunization records in-person at a KFL&A Public Health office during regular business hours.
